How does it work?
1. Alert on the Implicity platform
If a patient becomes disconnected from their manufacturer’s website, the medical team is notified through the Implicity platform. Providers can easily enable the SMS function, alerting patients immediately of their disconnection status.
2. Automatic text message sent to the patient
The patient receives a link via text message leading them to an application where they can enter the reason for their disconnection*.
3. Connectivity follow-up on the Implicity platform
The information provided by the patient is visible on the Implicity platform. The alert will automatically be snoozed until the date indicated by the patient. If the patient reconnects, the alert will be archived automatically on the Implicity platform, without any action from your side.
18.6% of the followed patients are disconnected**.
70% of medical centers have at least 10% of disconnected patients**.
Ensuring connectivity to remote transmitters is key to an optimal patient follow-up.
Managing disconnected patients is a time-consuming process with a low added value for the medical team.
How many opportunities do you miss due to disconnected patients?
Possible disconnection reasons:
- Technical issue with the transmitter
- Unplugged transmitter
- Other reasons
*Patients can also opt-out and disable the SMS option. In this case, the medical team receives an alert on the Implicity platform and the SMS option will be automatically de-activated.
**Observed on 22.105 patients with Medtronic and Abbott implanted cardiac devices, remotely monitored on 7 May 2021. Implicity internal data.
